Objectives for pharmacists              

  • Compare and contrast collaborative practice agreements (CPAs) and collaborative drug therapy modification (CDTM) and discuss their impact on patient care
  • Examine the process of obtaining a CDTM license and incorporating pharmacy residents into CDTM
  • Summarize healthcare providers’ perspectives on collaborative practice with pharmacists
  • Discuss factors impacting collaborative relationships with pharmacists and advanced practitioners

  • ·         Define collaborative practice agreements (CPAs) and collaborative drug therapy modification (CDTM)
  • ·         List the process of obtaining a CDTM certification
  • Recall healthcare providers’ perspectives on collaborative practice with pharmacists
  • Identify factors impacting collaborative relationships with pharmacists and advanced practitioners

This is a knowledge based activity.  This activity was planned to meet the educational needs of pharmacists and pharmacy technicians who practice in any setting.

Georgia Society of Health-System Pharmacists is accredited by the Accreditation Council for Pharmacy Education (ACPE) as a provider of continuing pharmacy education.   Participants of the session who complete the evaluation and provide accurate NABP e-Profile information will have their credit for 1.0 contact hours (0.10 CEU) submitted to CPE Monitor as early as 1 day after the event and no later than 60 days after the event.  Please know that if accurate e-Profile information is not provided within 60 days of the event, credit cannot be claimed after that time.  The participant is accountable for verifying the accurate posting of CE credit to their CPE Monitor account within 60 days. Participants must participate completely in the activity and complete the course evaluation in its entirety to claim continuing pharmacy education credit.

This is a member service of GSHP.  There is no charge for members to attend.  Dues must be current to receive CE credit.
